Community Health Worker

New Position

Catholic Charities Care Coordination Services - Schenectady Office

This position is an integral member of the interdisciplinary health home care management team. The position provides outreach, engagement and supportive care coordination activities to eligible clients enrolled in New York’s Medicaid Health Home program in Schenectady County. The program’s aim is to assist community members with one or more chronic illnesses get connected to health care and other needed services to become healthier and more self-sufficient, thereby reducing emergency room visits and inpatient hospital stays.
