COBRA Program
Case Manager
- Under the supervision of the CFP Supervisor, this position provides intensive case management activities to support Medicaid eligible clients enrolled in the Community Follow-up Program and their families in accessing needed services necessary to maintain their quality of life and promote independent functioning.
- Work activities include intake, assessment/reassessment, service planning, service plan monitoring and implementation.
- All client service contact is documented and billed to Medicaid.
- Educational requirements: Bachelor?s Degree and one year of qualifying experience, or Associate Degree and three years of experience required. Bi-lingual capabilities, knowledge of HIV/AIDS issues preferred.
Case Management Technician
- Under the supervision of the CFP Supervisor, this position assists the Case Manager in providing intensive case management activities to support Medicaid eligible clients enrolled in the Community Follow-up Program and their families in accessing needed services necessary to maintain their quality of life and promote independent functioning.
- Assist in gathering information for intake, assessment/reassessment, and service planning. Assist in service plan monitoring and implementation.
- Educational requirements: High school and two years of experience required, Associates preferred. Bi-lingual capabilities, knowledge of HIV/AIDS issues preferred.
Community Follow Up Worker
- Under the supervision of the CFP Supervisor, this position assists the Case Manager and Case Management Technician in providing intensive case management activities to support Medicaid eligible clients enrolled in the Community Follow-up Program, and their families, in accessing needed services necessary to maintain their quality of life and promote independent functioning.
- Assist in provision of services, clerical tasks relating to documentation, making and confirming appointments, arranging transportation and follow up of service provision.
- Educational requirements: High school degree or equivalent. Bi-lingual capabilities, knowledge of HIV/AIDS issues preferred.
General Job Requirements
All staff who work at in the COBRA Program at Catholic Charities AIDS Services should have:
- Familiarity with and sensitivity to specialized needs of people with HIV, of people with substance abuse and mental health issues, of people of color, of gay, lesbian, bisexual and transgender individuals. Ability to work with people of diverse backgrounds.
- Knowledge of HIV/AIDS and ability to provide clients with appropriate information.
- Familiarity with the cycle of poverty and working with families experiencing poverty.
- Familiarity with and ability to access community resources and services.
- Strong communication skills, including verbal ability, writing style and active listening skills.
- Attention to detail.
- Strong organizational skills including time management and ability to prioritize.
- Ability to work as a member of a team.
- Ability to provide strengths-based, client-centered, proactive case management.
- Ability to read/comprehend written and verbal instructions.
- Ability to manage stress appropriately, make decisions under pressure, handle multiple priorities, work alone and manage anger/fear/hostility/violence of others appropriately.
- Ability to interact with people from diverse backgrounds and provide an atmosphere of trust, mutual respect and caring.
- Must be able to perform light physical work. This position requires daily driving, walking, sitting and climbing as required to perform home visits and accompaniment with agency clients.
- Must have a valid driver's license, proof of current car insurance and registration and access to a working vehicle.
